Replace Fomantic's hard-forked jQuery modal with native `<dialog>`
elements, advancing https://github.com/go-gitea/gitea/issues/29849. The
UI is visually unchanged. Dialogs open with `show()` over an `inert`
page, so tooltips, popups and toasts mounted on `<body>` stay usable
above them, unlike with `showModal()`
(https://github.com/whatwg/html/issues/9936).
- Remove the Fomantic modal and dimmer, add `showModal` and `hideModal`
- Declare modals as `<dialog class="modal-dialog">`, shown inside a
`.modal-backdrop`, instead of `ui modal` with its unused size classes
- Remove two modals that could never be opened
- Block the page while a modal is open, close the topmost one on Escape
and restore focus to its opener
- Allow stacked modals, use a confirm modal instead of `window.confirm`
for deleting content history
- Hide popups and toasts when a modal opens or closes
- Keep the release notes modal open when generating notes fails

Follow-up to https://github.com/go-gitea/gitea/pull/37571.
"Participating and mentions" deleted the watch row, so choosing it
dropped you out of the watcher count. It is a watch like the others, so
it now keeps a row and simply subscribes to no events.
The dashboard feed ignored the per-event options, so a "Custom: issues"
watcher still got pull request activity there. It now gates on the same
options as mail and notifications. That also closes a gap where pull
request reviews bypassed the permission check.

Make the watch, star, and fork buttons in the repo header consistent for
logged-out users:
- Apply the same look to all three buttons (number labels
included), instead of only the action button being grayed.
- Clicking any of them while logged out now leads to the login page
(with a redirect back) instead of being inert.
- Split the per-button markup out of `header.tmpl` into a dedicated
`templates/repo/header/` folder (`fork.tmpl`, `star.tmpl`,
`watch.tmpl`).
